Applicant Authorizations
Read carefully before signing:
[1] If hired, I agree to comply with all the rules of this Company. I hereby affirm that the information provided on this application (and accompanying resume, if any) and in my interview, should there be one, is true and complete to the best of my knowledge and agree that false or misleading information or omissions may disqualify me from further consideration for employment and may be considered justification for dismissal if discovered at a later date.
[2] I hereby acknowledge that any employment relationship with this Company is of an “at will” nature, which means that I may resign at any time, and the Company may discharge me at any time with or without cause. It is further understood that this “at will” employment relationship may not be changed by any written document or by conduct unless such change is specifically acknowledged as applying to me in writing and signed by the President of the Company.
[3] I authorize all individuals, companies and institutions named in this application (and accompanying resume, if any) and all individuals connected therewith, to provide Cooperative Building Solutions and any representative or agent thereof with any relevant information they may have concerning my character, health and employment record which Cooperative Building Solutions may require to arrive at an employment decision. I hereby release them from all liability for any damage whatsoever of issuing same. If I am applying for a job which might require the operation of a vehicle, I hereby authorize the Company to obtain any and all information pertaining to my driving record.
[4] I understand that a routine inquiry may be made during Cooperative Building Solutions initial or subsequent processing of my employment application into my character, general reputation and personal characteristics.
[5] I agree to take a complete physical examination after an offer of employment has been extended and prior to hire, and at anytime during my employment if the Company requires me to do so. I understand that the examining physician or facility may disclose to the Company or its representatives the results of such examination subject to its use on a need-to-know basis, and that the medical records will be maintained separately from employment records. I understand that my continued employment may be conditioned on the findings of this examination. I agree, if the Company requires me to do so, to undergo a comprehensive drug test prior to hire and drug/alcohol tests at any time during my employment at the discretion and expense of the company. I agree to sign the required forms authorizing such testing and permitting the examining laboratory to disclose to the Company and its representatives the results of such tests to be included in my medical records. I understand my initial and continued employment is conditioned on my consent to such testing as well as the findings/results of the tests.
[6] I understand that my employment may be terminated by me or the Company at any time without advance notice one to the other, should I prove unsatisfactory for any reason or should there be a reduction of the work force. I understand that if I am hired into or transferred to a position created by leave of absence granted an employee in accordance with the several statues governing re-employment rights of employees (military, maternity, etc.) and the employee on leave of absence returns to work, I will be transferred, reclassified, or released, as the management of the Company shall, in its sole discretion, determine to be appropriate.
[7] In consideration of my employment by Cooperative Building Solutions, I irrevocably consent to the use by Cooperative Building Solutions of my name, picture, or likeness for any advertising, publicity, or other legitimate business purpose, regardless of whether I am employed by Cooperative Building Solutions when the name, picture or likeness is used. I irrevocably waive any rights of privacy and publicity, any property rights, and any other rights and claims arising out of Cooperative Building Solutions legitimate use of my name, picture or likeness. I irrevocably waive any additional consideration, compensation, or benefit in the event my name, picture, or likeness is used. Any negatives, prints, videotape, or other material for printing or reproduction in connection with the use of my name, picture, or likeness will be Cooperative Building Solutions property.
